University of Memphis/LiFE - Student Agreement *

I understand and agree that:

The tuition benefit is limited to the fiscal amount authorized by FedEx.
The tuition benefit may be used toward a degree offered through UofM Global only.
The tuition benefit applies to employees approved by FedEx.
Federal and/or state grants, and/or scholarships will be applied to my balance prior to the tuition benefit offered by FedEx.
To have my tuition paid by FedEx I must fulfill each of the following conditions:
- Undergraduate students must complete a current year Free Application for Federal Student Aid (FAFSA) and all additional procedures required by the Financial Aid office.
- Enroll, complete, and earn a final grade that meets graduation requirements for my approved degree.
- Notify a Student Advocate (lifebyglobal@memphis.edu) that an eligible grade has been posted for an Incomplete, Missing Grade, and/or Grade Change after the semester of record within 60 days of the official posting of the grade change. The tuition benefit will be applied after the course work has been successfully completed in accordance with the academic policy and I have fulfilled the other student agreement requirements for the semester of record in which I was enrolled, and I am employed at the time I request tuition payment.
- Commit to a degree that can be completed entirely online and is available as part of the UofM Global portfolio.
- Earn a grade in each course that meets graduation requirements for my chosen degree.
  - Furthermore, I understand that I will assume responsibility for all fees and tuition for all coursework in which my final grade does not meet graduation requirements. Disqualifying final grades may vary by course and may include, but are not limited to, D, F, Withdrawal, Audit and Incomplete.

I authorize:

The release of information about my grades, financial aid, academic progress and FedEx employment record to The University of Memphis.
The release of information about my grades, financial aid, and academic progress to FedEx.

I understand and agree that I:

Am accountable for all communication and program information.
Am responsible for course registration each semester, including dropping classes if I no longer plan to participate in the program.
Will comply with all rules of the LiFE program and requirements of University of Memphis.
Will be bound by the University of Memphis' Code of Student Conduct.
Will be disqualified from this agreement if I terminate employment, if I accept a position that is no longer LiFE eligible, or if I am dismissed for any reason by FedEx or the University of Memphis.
Understand that FedEx and The University of Memphis reserve the right to change terms, conditions, and benefits.
Understand that I may only participate in one FedEx tuition benefit program in any given semester.
Assume responsibility for all fees and tuition if I terminate employment, if I accept a position that is no longer LiFE eligible, if I am dismissed for any reason by FedEx or the University of Memphis, if I do not choose an approved degree do not enroll as a Global student, or I fail to earn the minimum grade as outlined by this agreement.
