Diploma Reorder Form

This process is for diploma reorders ONLY.  If you have not received your original diploma and it has been more than three months since your commencement, please contact the Commencement Office.

All diplomas will read University of Memphis ONLY.

There is a $20 fee for diploma reorders. This fee is non-refundable. Once you complete the form you will have the option to pay on-line or print the form and send it in with payment.

Once we receive your reorder request, it will take approximately three and a half weeks for our vendor to process your diploma. When we receive your diploma from our vendor we will mail it to you as you specify below (standard or expedited).

FedEx overnight shipping is non-refundable.  No overnight shipping is available to Hawaii, Alaska or internationally.

Diplomas that are damaged during shipping or that must be corrected due to a printing error may be returned for a replacement at no charge.
